Cocoa Flavanols

Cocoa flavanols reliably improve endothelial function and shave a couple of mmHg off blood pressure. The large COSMOS outcome trial, however, missed its primary cardiovascular endpoint — the biomarker benefits did not clearly convert into fewer events.

What the science says

  • Lowers blood pressure Moderate evidence

    A Cochrane review of 35 randomised trials (1,804 participants) found cocoa reduced systolic blood pressure by about 1.8 mmHg and diastolic by about 1.8 mmHg over 2-18 weeks. The effect is small, larger in hypertensive participants than normotensive ones, and the evidence was graded moderate quality with unblinded taste as a persistent limitation.

  • Improves endothelial function Strong evidence

    Flow-mediated dilation improves consistently and substantially — pooled increases of roughly 1.3-2 percentage points, both acutely (2 hours post-dose) and chronically. This is one of the more reproducible vascular effects of any polyphenol, and the mechanism (epicatechin-driven nitric oxide bioavailability) is well characterised.

  • Prevents cardiovascular events Limited evidence

    COSMOS randomised 21,442 older adults to cocoa extract (500 mg flavanols, 80 mg epicatechin daily) or placebo for a median 3.6 years. The primary composite cardiovascular endpoint was not significantly reduced (HR 0.90, 95% CI 0.78-1.02). A prespecified secondary analysis showed reduced cardiovascular death (HR 0.73), but as a secondary endpoint in a null primary trial this cannot be treated as established.

  • Improves cognition Limited evidence

    Small trials report modest gains in executive function and cerebral blood flow, but the COSMOS-Web cognitive substudy found no overall benefit of cocoa extract on global cognition over 2 years. Cocoa is not a demonstrated cognitive enhancer.

Dosage & safety

Studied doseTrials used 400-1,000 mg/day of cocoa flavanols, most commonly around 500 mg/day providing roughly 80 mg epicatechin (the COSMOS dose), taken as a standardised extract in divided doses. Achieving this from chocolate is impractical: dark chocolate flavanol content varies enormously and alkalised (Dutch-processed) cocoa loses most of it, so hitting 500 mg could mean hundreds of calories of sugar and fat. Vascular effects appear acutely within 2 hours; blood pressure effects take 2 weeks or more.
SafetyCocoa extract was well tolerated in COSMOS over 3.6 years, with mild gastrointestinal complaints the main difference from placebo. Cocoa contains caffeine and theobromine — high-dose extracts can cause jitteriness, palpitations or sleep disruption in sensitive people, so avoid late-day dosing. Cocoa products, including extracts, can carry cadmium and lead; look for third-party heavy metal testing, particularly for daily long-term use. Theobromine is toxic to dogs — keep away from pets. May mildly potentiate antihypertensives. Chocolate as a delivery vehicle brings sugar and calories that can outweigh any vascular benefit.
