AREDS2 Eye Formula

One of the few supplement formulas with a large, government-funded RCT behind it — but only for a specific population: people who already have intermediate age-related macular degeneration, or advanced AMD in one eye. It slows progression modestly; it does not prevent AMD in healthy eyes or improve vision.

What the science says

  • Slows progression to advanced AMD in people with intermediate disease Strong evidence

    In AREDS2 (n=4,203, median 5 years), the 10-year follow-on found a hazard ratio of 0.91 (95% CI 0.84-0.99) for progression to late AMD with lutein/zeaxanthin, and 0.80 (0.68-0.92) when restricted to those who had been assigned beta carotene. The 2023 Cochrane review concludes that antioxidant vitamin and mineral supplementation probably slows progression to late AMD in people who already have the disease — an approximately 20-25% relative risk reduction over five years, not a cure.

  • Lutein/zeaxanthin is a safer swap for beta carotene Strong evidence

    AREDS2 was designed partly to remove beta carotene, which had raised lung cancer risk in smokers in earlier trials. In the 10-year follow-on, beta carotene assignment carried an odds ratio of 1.82 (95% CI 1.06-3.12) for lung cancer — nearly double — versus 1.15 (0.79-1.66, non-significant) for lutein/zeaxanthin. Any AREDS product still containing beta carotene should be avoided by current or former smokers.

  • Adding omega-3 fatty acids helps Limited evidence

    AREDS2 tested DHA 350 mg plus EPA 650 mg daily and found no effect on AMD progression; at 10 years the hazard ratio was 1.01 (95% CI 0.93-1.09, p=0.91). Products marketing an 'AREDS2 plus omega-3' upgrade are selling an intervention the trial specifically tested and found inert for this endpoint.

  • Prevents AMD or cataracts in healthy eyes Limited evidence

    AREDS2 enrolled only people at high risk who already had AMD, so nothing in the trial supports prevention in people with normal retinas. The parallel AREDS2 cataract report found lutein/zeaxanthin did not reduce the rate of cataract surgery overall. Taking it 'for eye health' with no diagnosis is unsupported and, with 80 mg zinc, not risk-free.

Dosage & safety

Studied doseThe AREDS2 formula is vitamin C 500 mg, vitamin E 400 IU, lutein 10 mg, zeaxanthin 2 mg, zinc 80 mg (as zinc oxide) and copper 2 mg (as cupric oxide), taken daily in divided doses, indefinitely. A lower-zinc version (25 mg) performed no differently in the trial (HR 1.04 for low vs high zinc) and is a reasonable choice for people who find 80 mg hard on the stomach. Verify the label matches these amounts — many 'eye health' products carry the AREDS name with substituted or reduced ingredients.
SafetyThe 80 mg zinc dose is far above the 40 mg tolerable upper limit and causes nausea and copper depletion, which is why copper 2 mg is included; long-term high-dose zinc has been associated with genitourinary hospitalisations in AREDS. Vitamin E at 400 IU has been linked in other trials to increased haemorrhagic stroke risk and, in SELECT, to increased prostate cancer incidence — a reason not to take AREDS2 without an actual AMD diagnosis. Do not take an AREDS formula that still contains beta carotene if you have ever smoked. Consult your ophthalmologist before starting, and do not use it as a reason to skip retinal monitoring or anti-VEGF treatment for wet AMD.
