Best supplements for IBS (irritable bowel syndrome)

IBS should be diagnosed by a doctor before you self-treat, and bleeding, weight loss or anaemia need prompt attention. Enteric-coated peppermint oil is the only supplement with a strong grade for IBS, while psyllium, a gentle fiber and a few named probiotic strains add moderate evidence. Picks are ranked by the grade for IBS symptoms, and with probiotics the strain on the label matters more than the species.

Picks are ranked by the strength of published evidence for this goal in otherwise healthy adults. A persistent symptom needs a clinician's evaluation, and no supplement on this list replaces treatment.

Peppermint Oil (Enteric-Coated) Strong evidence abdominal pain and global symptoms In a meta-analysis of 10 trials it beat placebo for global IBS symptoms with a number needed to treat of about 4, and it also eased abdominal pain. Use enteric-coated capsules at 180–225 mg two to three times daily; heartburn is the main side effect, so be cautious with reflux. View

Psyllium Husk (fiber) Strong evidence the best-studied fiber for IBS A 2026 meta-analysis of 30 trials found 52% responded to fiber versus 44% on control, with psyllium the type that clearly helped. Start at 5 g a day in plenty of water, and take medicines 1 hour before or 2–4 hours after. View
Partially Hydrolyzed Guar Gum Moderate evidence fiber with less gas Small trials found it improved IBS symptom scores and was at least as good as wheat bran with fewer complaints. Trials used about 5–7 g a day stirred into a drink. View
Lactobacillus Plantarum Moderate evidence bloating and pain (strain 299v) Trials of strain 299v at 10–20 billion CFU a day for 4 weeks improved bloating and pain, with response rates typically 15–25 percentage points above placebo. Buy a product that lists the strain code, not generic L. plantarum. View
Bifidobacterium Infantis Moderate evidence global symptoms (strain 35624) In a trial of 362 women, strain 35624 at 100 million CFU a day improved pain and bloating while higher and lower doses did not. Judge it after 4 weeks. View
Bifidobacterium Bifidum Moderate evidence replicated result (strain MIMBb75) Strain MIMBb75 improved IBS symptoms and quality of life in two randomized trials, one using a heat-inactivated version, which is unusual replication for a probiotic. Trials used about 1 billion CFU a day for 4–8 weeks. View

Bacillus Coagulans Moderate evidence diarrhea-predominant IBS Small trials of strain MTCC 5856 at about 2 billion CFU a day improved symptom scores in diarrhea-predominant IBS. Several share the same manufacturer, so treat it as a second choice. View

Popular for this goal — but the evidence says skip

  • Inulin (Prebiotic Fiber) Moderate evidence

A 2019 meta-analysis of prebiotics found no overall benefit for IBS, and as a FODMAP inulin more often triggers gas and bloating than relieves them.

  • Zinc Carnosine Moderate evidence

IBS and gut healing drive its sales, but those uses have essentially no randomized trial support.

  • Artichoke Extract Moderate evidence

The IBS claim rests on a post-hoc subgroup of an open-label dyspepsia study, never confirmed against placebo.

  • Aloe Vera Moderate evidence

Controlled IBS data are a handful of small trials with mixed results, and the laxative latex was pulled from US over-the-counter laxatives in 2002 for lack of safety data.

  • Digestive Enzymes Limited evidence

Broad enzyme blends have few placebo-controlled trials, and a normal pancreas already makes far more enzyme than a capsule supplies.

  • Slippery Elm Limited evidence

The only IBS study was uncontrolled and used formulas with up to five other herbs, so it cannot support a slippery elm claim.

Our verdict

Start with enteric-coated peppermint oil for 4–12 weeks, and add psyllium or partially hydrolyzed guar gum if you want a fiber, building up slowly. If you try a probiotic, buy the exact studied strain and stop after 4 weeks if nothing changes. Skip inulin, zinc carnosine, artichoke, aloe and broad enzyme blends.
