Mangosteen

Mangosteen pericarp xanthones are pharmacologically interesting and mangosteen gel has decent data as a local periodontal treatment — but the juice sold through multi-level marketing for "immune and inflammation support" has essentially no oral-supplement evidence behind it.

What the science says

  • Improves periodontal health (as a local gel) Moderate evidence

    A meta-analysis of randomized trials using mangosteen gel as local drug delivery into periodontal pockets found significant reductions in probing pocket depth (effect size -1.08, 95% CI -1.40 to -0.77) and clinical attachment level (-0.96, 95% CI -1.80 to -0.13). Note what this is: a gel placed in a gum pocket by a dentist. It says nothing about drinking mangosteen juice.

  • Supports cognition in Alzheimer's disease Limited evidence

    A 24-week RCT (n = 108) of water-soluble mangosteen pericarp extract at 4–8 mg/kg/day found a higher proportion of low-dose participants reaching the minimum clinically important ADAS-Cog change versus placebo, with a fall in the oxidative marker 4-hydroxynonenal. The trial was small, single-centre, and responder-analysis based; it needs replication before it means anything clinically.

  • Improves blood sugar in type 2 diabetes Limited evidence

    A 2026 systematic review of human studies of mangosteen peel extract in type 2 diabetes found only a handful of small, heterogeneous trials with inconsistent glycemic effects and no pooled benefit that could be relied on.

  • Antioxidant "superjuice" for immunity and inflammation Limited evidence

    Commercial mangosteen juices are the product this fruit is famous for, and they are the least studied form. Xanthone content is usually undeclared and low relative to trial doses of pericarp extract, and no trial has tested a marketed juice against a clinical endpoint.

Dosage & safety

Studied doseOral trials used 4–8 mg/kg/day of standardised water-soluble pericarp extract (roughly 280–560 mg/day for a 70 kg adult), or 400–1000 mg/day of pericarp extract in metabolic studies. Periodontal benefit came from a 4% gel applied subgingivally, not from anything swallowed.
SafetyGenerally well tolerated in trials. A published case report links prolonged daily mangosteen juice use to severe lactic acidosis. Alpha-mangostin shows antiplatelet activity in vitro, so a theoretical bleeding risk exists with warfarin, DOACs or antiplatelet drugs and around surgery. Pericarp is very high in tannins, which can cause GI upset and reduce non-heme iron absorption. The FDA has issued warning letters over unapproved disease claims for mangosteen juice products; treat cancer, arthritis and immune "cure" marketing as unsupported.
