A meta-analysis of randomized trials found ~1,000 mg/day of curcumin reduced arthritis pain significantly versus placebo, with some trials showing effects comparable to NSAIDs — though studies are short and mostly small.

Curcumin lowers some inflammatory markers in trials, but plain curcumin is very poorly absorbed — benefits depend on enhanced formulations (with piperine or phospholipids), and long-term clinical outcomes are unproven.

- warfarin, DOACs and antiplatelet drugs
Curcumin has antiplatelet activity and case reports describe raised INR on warfarin. do not combine without medical supervision; stop 2 weeks before surgery
- narrow-therapeutic-index drugs, especially with piperine-containing formulas
Piperine inhibits drug metabolism and transport, raising blood levels of drugs such as phenytoin, propranolol, theophylline and tacrolimus. choose a piperine-free formulation, or check with your pharmacist before combining
- chemotherapy and tamoxifen
Curcumin affects the CYP enzymes and transporters several cancer drugs depend on, in both directions. do not use during cancer treatment without your oncologist's agreement
- insulin and other antidiabetic drugs
Modest additive glucose lowering. monitor blood glucose when starting
With other supplements
- iron
Curcumin chelates iron and can worsen iron status in people who are already marginal. separate from iron supplements and avoid high doses if you are iron deficient
