HMOs (2'-Fucosyllactose)
The main sugar of human breast milk, now sold as an adult prebiotic. It reliably shifts the adult microbiome toward bifidobacteria, but the few trials in adults show inconsistent clinical benefit.

What the science says
- Microbiome modulation in adults Moderate evidence
A randomized controlled trial in older adults showed that a human milk oligosaccharide changed the microbiome along with circulating hormones and metabolites. A separate randomized, double-blind trial found 2'-FL produced only a transient improvement in microbial resilience after antibiotic treatment.
- IBS symptoms Limited evidence
A parallel, randomized, double-blind, placebo-controlled study of HMO supplementation in IBS patients did not show clear symptom benefit over placebo, despite promising open-label results beforehand.
- Infant gut development Moderate evidence
The strongest evidence for 2'-FL is in infant formula, where it shifts the infant microbiome toward a breastfed pattern. Extrapolating that to adults is not justified by the adult trials.
