Graviola (Soursop)

Heavily marketed as a natural cancer treatment on the basis of cell-culture data, graviola has never been tested for cancer in a human trial — and its signature compounds are the leading suspect in an epidemic of atypical parkinsonism in Guadeloupe. This is a page about why not to take it.

  • Treats or prevents cancer Limited evidence

    Annonaceous acetogenins from Annona muricata kill a wide range of cancer cell lines in vitro at sub-micromolar concentrations by inhibiting mitochondrial complex I, and shrink tumors in some xenograft models — this is the entire basis of the 'graviola kills cancer cells 10,000 times better than chemo' marketing. There is no published randomized controlled trial, and no controlled clinical trial of any design, of graviola for any cancer in humans. Cytotoxicity in a dish is a starting point for drug discovery, not evidence of a treatment.

  • Linked to atypical parkinsonism Moderate evidence

    Guadeloupe has an unusual cluster of atypical parkinsonism, and case-control work found consumption of Annonaceae fruit and herbal teas — soursop prominently among them — associated with the disease, with the acetogenin annonacin as the leading candidate. Annonacin given systemically to rats causes nigral and striatal neurodegeneration at doses producing plasma levels comparable to lifelong dietary exposure. The association comes from heavy, chronic, lifelong consumption, but concentrated leaf extracts deliver acetogenins at doses no traditional diet ever achieved.

  • Safe and well tolerated Limited evidence

    A systematic review of the safety of Annona muricata leaf extract concluded the overall profile is favourable and worth further study, while explicitly listing neurotoxic effects among the findings and noting that animal doses and durations may not translate to humans. That is a careful statement about a thin literature, not a clean bill of health — the reviewed evidence is largely in vitro and preclinical, with almost no human dosing data.

Dosage & safety

Studied doseNo human dose has been established for any indication. Commercial products range from 500-1000 mg of leaf powder up to concentrated 4:1 and 10:1 extracts, with acetogenin content essentially never disclosed. Fruit pulp eaten occasionally as food is a different exposure from a standardized leaf extract taken daily, and the concentrated extracts are precisely the products the neurotoxicity concern applies to.
SafetyThe central concern is annonacin-driven neurotoxicity: chronic exposure is epidemiologically linked to atypical parkinsonism and PSP-like syndromes in Guadeloupe, and annonacin reproduces nigrostriatal degeneration in rats. Avoid concentrated leaf extracts entirely, and treat daily long-term use as unjustifiable given zero proven benefit. Graviola also has hypotensive and hypoglycemic activity in animals, creating additive risk with antihypertensives and diabetes medication, and cases of movement disorder have been reported with heavy consumption. It should never be used in place of oncology treatment; patients who delay conventional therapy for unproven botanicals have worse survival. Avoid in pregnancy, in Parkinson's disease, and with any neurodegenerative diagnosis.
