Flaxseed Oil

Flaxseed oil is a concentrated source of ALA, a plant omega-3, but humans convert almost none of it to EPA and DHA. Whole or ground flaxseed has better evidence for cholesterol lowering than the oil, because the fibre and lignans do most of the work.

What the science says

  • Not a substitute for fish or algal omega-3 Strong evidence

    Conversion of ALA to EPA is roughly 5-8% and to DHA typically under 1% in adults, so flaxseed oil does not meaningfully raise the omega-3 index. If the goal is DHA status, algal oil or fish oil is required.

  • Small effects on blood lipids Limited evidence

    A meta-analysis of 28 flaxseed intervention trials found significant reductions in total cholesterol (about 0.10 mmol/L) and LDL (about 0.08 mmol/L), but the effect was driven by whole flaxseed and lignan preparations — flaxseed oil specifically showed no significant lipid change. A separate meta-analysis of ALA supplementation found only very small triglyceride reductions.

  • Cardiovascular outcome benefit Limited evidence

    Observational data associate higher ALA intake with lower cardiovascular risk, but no randomized outcome trial of flaxseed oil has shown reduced cardiac events. Treat any claim of heart-attack prevention from flaxseed oil as unproven.

Dosage & safety

Studied doseTypical supplement doses are 1-3 g/day of oil (roughly 0.5-1.7 g ALA per gram). Trials showing lipid benefit generally used 30-50 g/day of ground whole flaxseed, not the oil. If cholesterol lowering is the aim, ground flaxseed is the better-supported form.
SafetyWell tolerated; loose stools and mild GI upset are the usual complaints. The oil oxidizes rapidly — refrigerate, buy small bottles, and discard if it smells rancid. Do not heat or cook with it. Mild antiplatelet effect at high doses; use caution with anticoagulants. Whole flaxseed (not the oil) contains lignans with weak phytoestrogen activity, relevant to people with hormone-sensitive cancers who should discuss it with their oncologist. Flaxseed can also delay absorption of oral medications taken at the same time.
