Rutin

The rutin in supplement bottles is not the rutin in the studies. What has real vein evidence is hydroxyethylrutosides (oxerutins) — semi-synthetic derivatives sold as medicines in Europe — while plain rutin is poorly absorbed and barely tested on its own.

What the science says

  • Hydroxyethylrutosides modestly improve venous insufficiency symptoms — plain rutin has not been shown to Moderate evidence

    A systematic review of 15 trials (1,643 participants) of hydroxyethylrutosides versus placebo found reduced pain (SMD -1.07, 95% CI -1.44 to -0.70), fewer heavy-leg symptoms (OR 0.50, 95% CI 0.28-0.91) and fewer cramps (SMD -1.07), with no serious adverse effects — but the authors judged every included trial to be of limited quality. Rutosides were also the single largest phlebotonic subgroup (28 trials) in the 2020 Cochrane review, which found only a slight oedema reduction (RR 0.70) and no quality-of-life benefit for the class.

  • Plain rutin is a poorly absorbed way to get quercetin Moderate evidence

    Rutin is quercetin-3-rutinoside: like hesperidin, the rhamnose-glucose sugar must be cleaved by colonic bacteria before absorption, so plasma quercetin metabolites after a rutin dose are substantially lower and slower than after the same dose of quercetin aglycone. If your goal is quercetin exposure, buying rutin wastes most of the dose.

  • The "vitamin P" and capillary-fragility story is obsolete Limited evidence

    Rutin was named vitamin P in the 1930s; the designation was withdrawn because no deficiency state exists and it is not essential. There are no adequately powered modern trials supporting rutin alone for easy bruising, capillary fragility, hemorrhoids, or antioxidant health outcomes at the 250-500 mg doses sold.

Dosage & safety

Studied doseOxerutin/hydroxyethylrutoside trials used 1000-2000 mg/day (typically 1 g twice daily) for 4-12 weeks. Plain rutin supplements are usually 250-500 mg once to three times daily — doses with no trial support for any specific outcome.
SafetyMild and infrequent: GI upset, headache, flushing, rash. Rutin has weak anticoagulant/antiplatelet activity in laboratory and animal models, so use caution alongside warfarin, DOACs or aspirin and stop before surgery. Very high intravenous rutoside doses have caused rare hypersensitivity reactions; oral use has not. No long-term human safety data. Not established in pregnancy or breastfeeding. Not banned in sport.
