Manuka Honey

Manuka honey has a genuinely distinctive antibacterial chemistry (methylglyoxal) and medical-grade versions are legitimate wound dressings, but the evidence that eating it does anything special — for immunity, gut health, or colds — is close to absent. The oral price premium over ordinary honey is not evidence-based.

What the science says

  • Antibacterial activity in vitro is real and dose-graded Strong evidence

    Manuka's non-peroxide activity comes from methylglyoxal (MGO), formed from dihydroxyacetone in Leptospermum scoparium nectar; UMF/MGO grades track MGO concentration (e.g. UMF 10+ ≈ 250 mg/kg MGO, UMF 20+ ≈ 800 mg/kg). Laboratory work shows inhibition of Staphylococcus aureus including MRSA, Pseudomonas aeruginosa, and biofilms at concentrations achievable topically. These are petri-dish and ex vivo concentrations, and do not transfer to what reaches tissue after swallowing a teaspoon.

  • Topical wound healing Moderate evidence

    A Cochrane review of 26 trials (n≈3,011) found honey probably shortens healing of partial-thickness burns versus conventional dressings (mean difference roughly 4-5 days) and heals infected post-operative wounds faster than antiseptic plus gauze, but evidence for leg ulcers, diabetic foot ulcers, and most other wound types was low-certainty or showed no benefit. Most trials used honey generically rather than manuka specifically, so this is an argument for medical-grade honey dressings, not for the retail jar.

  • Eating manuka honey for immunity, colds, or H. pylori Limited evidence

    There is no convincing RCT evidence that oral manuka improves immune markers, prevents respiratory infections, or eradicates Helicobacter pylori; the H. pylori claim rests on in vitro inhibition that repeated human attempts have failed to reproduce. Honey of any type has modest evidence for suppressing acute cough in children, which is a demulcent effect and not manuka-specific.

  • Gut microbiome and 'prebiotic' benefit Limited evidence

    Manuka contains oligosaccharides that ferment in vitro, but no controlled human trial shows a meaningful microbiome or symptom benefit at realistic intakes. At 1-2 teaspoons you are consuming about 8-16 g of mostly fructose and glucose — the sugar load is the more certain physiological effect.

Dosage & safety

Studied doseCulinary/oral use is typically 1-2 teaspoons (7-15 g) daily; no oral dose has been established as effective for any outcome. Wound care uses medical-grade, gamma-irradiated honey applied as a dressing — not food-grade jar honey, which is not sterile and may contain Clostridium spores. UMF/MGO grading is a chemistry label, not a clinical potency claim.
SafetyGenerally safe as a food. Never give any honey, manuka included, to infants under 12 months due to infant botulism risk. It is 80%+ sugar: daily use meaningfully raises glycemic load and is a poor choice for people with diabetes or those tracking added sugar. Do not apply food-grade honey to open wounds — use only sterile medical-grade products, and avoid honey dressings in people with known bee or honey allergy. Stinging on application is common with wound use. Marketing regularly outruns the evidence: authenticity fraud is well documented, so UMF/MGO certification matters for getting what you paid for, not for proving a health effect.
