Fructooligosaccharides (FOS)

A cheap, widely used prebiotic that feeds bifidobacteria. The microbiome shift is real, but clinical benefits in adults are small and most of the meta-analytic evidence comes from infant formula mixtures rather than FOS on its own.

What the science says

  • Shift in gut microbiota Moderate evidence

    A randomized, double-blind, placebo-controlled trial in overweight prediabetic adults showed that short-chain FOS changed the composition of the gut microbiota, with only modest accompanying metabolic changes.

  • Infant gut and stool outcomes Moderate evidence

    A systematic review with meta-analysis of formulas containing a 9:1 mixture of short-chain GOS and long-chain FOS reported benefits for gastrointestinal outcomes in healthy infants and toddlers. The result belongs to the mixture, not to FOS alone.

  • Constipation Limited evidence

    A randomized, double-blind trial in older adults with functional constipation used FOS combined with a Bifidobacterium lactis strain and reported improvement, so the contribution of FOS by itself cannot be separated out.

Dosage & safety

Studied doseStudies used roughly 5–10 g per day; many products supply 2–5 g per serving. Increase gradually.
SafetyAmong the most gas-producing prebiotics; bloating, cramping and flatulence are frequent above ~5 g/day. A FODMAP, so poorly tolerated in IBS and best avoided in SIBO and in fructose intolerance.
