Folinic Acid
A reduced folate that bypasses the MTHFR step and reaches the brain when the folate receptor is blocked. The actual trial evidence is in autistic children with language impairment — positive, but small and largely from one research group.

What the science says
- Verbal communication in autism with language impairment Moderate evidence
A 12-week double-blind placebo-controlled trial in 48 children found high-dose folinic acid improved verbal communication (medium-to-large effect, d=0.70), with the biggest gains in children positive for folate receptor-alpha autoantibodies (d=0.91).
- Replicated in a second trial and pooled analysis Moderate evidence
A separate randomized placebo-controlled trial found folinic acid as add-on therapy reduced inappropriate speech in autistic children, and a 2021 systematic review and meta-analysis of 21 treatment studies (including 4 placebo-controlled) reported improvements in communication and core symptoms.

The reason most people buy folinic acid — that MTHFR gene variants require a reduced folate — has not been tested in any trial of folinic acid in healthy adults with MTHFR polymorphisms.
