SIBO Herbal Formulas

Multi-herb antimicrobial formulas, usually berberine plus oregano or thyme oil. Berberine on its own has genuine trial support in SIBO; the packaged blends have only one non-randomized comparison behind them.

What the science says

  • Berberine ranks well in a network meta-analysis Moderate evidence

    A 2025 Bayesian network meta-analysis of 30 RCTs (1,552 participants, 12 interventions) ranked berberine monotherapy highest for eradication in uncomplicated SIBO. The authors present the ranking as hypothesis-generating rather than settled.

  • The blends themselves are barely tested Limited evidence

    The study usually cited is a retrospective, non-randomized chart review in which 46% of 37 patients on herbal therapy normalised their breath test versus 34% of 67 on rifaximin — a difference that was not statistically significant.

  • No placebo-controlled trial of a commercial blend Limited evidence

    Without a placebo arm it is impossible to separate the herbs from the effect of time, dietary change and the substantial test-retest variability of lactulose breath testing.

Dosage & safety

Studied doseBerberine trials used roughly 400–1,500 mg/day for about four weeks. Commercial blends follow label directions, typically two capsules twice daily for 4–8 weeks.
SafetyBerberine inhibits CYP3A4 and P-glycoprotein and can raise blood levels of many drugs, including statins and ciclosporin; it also lowers blood glucose. Avoid in pregnancy, breastfeeding and in newborns (risk of kernicterus). Oregano oil can irritate the gut lining. SIBO should be confirmed rather than assumed.
