B vitamins plus alpha-lipoic acid marketed for neuropathy. Alpha-lipoic acid has meta-analysis support for diabetic nerve symptoms; the B vitamin part mainly helps if you are deficient.

What the science says
- Alpha-lipoic acid for diabetic polyneuropathy Moderate evidence
A systematic review and meta-analysis found that alpha-lipoic acid supplementation improved clinical neuropathy scores and inflammatory markers in patients with diabetic polyneuropathy. Effects on symptoms are clearer than effects on nerve conduction, and most trials ran only a few months.
- Compared with other disease-modifying options Moderate evidence
A systematic review and meta-analysis of disease-modifying therapies for diabetic peripheral neuropathy included alpha-lipoic acid among the agents with symptom benefit, while noting that no supplement has been shown to reverse established nerve damage.
- B vitamins beyond deficiency Limited evidence
B12 and benfotiamine correct a real cause of neuropathy when deficiency is present — a common situation in people on long-term metformin — but there is little evidence that adding them helps nerve symptoms in people with normal levels.
