L-Carnosine

Carnosine is the beta-alanine/histidine dipeptide that beta-alanine supplements exist to build in muscle. Taking carnosine directly works too, but serum carnosinase splits most of it before it reaches tissue — so for muscle buffering, beta-alanine is the cheaper and better-validated route. Direct carnosine has its own modest signals for glycemic control and memory.

What the science says

  • Modest improvements in glycemic control and insulin resistance Moderate evidence

    A 2021 systematic review and meta-analysis of carnosine or beta-alanine supplementation found significant reductions in HbA1c and improvements in insulin resistance measures in human trials, with the clearest effects in people who already had dysglycemia or overweight. Effects in healthy normoglycemic adults were small to null, and the human trial base was modest in size.

  • Small but significant improvement in delayed recall Moderate evidence

    A 2024 systematic review and meta-analysis of histidine-containing dipeptide supplementation (carnosine and anserine) pooled randomized trials and found a significant improvement in delayed recall, with no consistent effect on other cognitive domains. Doses were typically 500-2,000 mg/day over 8-14 weeks, and several trials were in older adults or people with mild cognitive impairment.

  • For muscle carnosine loading, beta-alanine is the better buy Strong evidence

    Oral carnosine is hydrolyzed to beta-alanine and histidine by serum carnosinase, meaning much of a carnosine dose reaches muscle as beta-alanine anyway — while costing far more per gram. Beta-alanine at 3.2-6.4 g/day for 4+ weeks reliably raises muscle carnosine 40-80% and has an ISSN position stand and multiple meta-analyses behind its performance effect. If your goal is high-intensity exercise buffering, buy beta-alanine.

Dosage & safety

Studied doseHuman trials used 500-2,000 mg/day of L-carnosine, usually split, for 8-14 weeks; glycemic trials clustered around 1-2 g/day. For exercise buffering use beta-alanine 3.2-6.4 g/day (split into 0.8-1.6 g doses or use sustained-release to limit paresthesia). Carnosine's effects, like beta-alanine's, accumulate over weeks — it is not an acute-dose supplement.
SafetyWell tolerated in trials; the main reports are mild GI upset. Unlike beta-alanine, direct carnosine rarely causes paresthesia (skin tingling) at typical doses, since dosing is lower. Serum carnosinase activity varies widely between individuals (and by CNDP1 genotype), so response is inconsistent. Not recommended in pregnancy or breastfeeding due to absent data. Zinc-carnosine is a different product with different (gastric) evidence — do not treat them as interchangeable. Not banned in sport.
